In this presentation, Dr. Nimish Mehta discusses the synthetic approach to prescribing in homeopathy. He emphasises that while synthetic prescribing is a powerful tool for understanding remedies and handling complex cases, it must be used with clinical discipline as a secondary option when standard classical methods fail to reveal a clear simillimum.

Key Principles

Roots in History: Traces synthetic prescribing back to James Tyler Kent’s 1908 study of compound salts. Despite historical pushback over the use of proving data, Kent used group characteristics and key symptoms to guide remedy selection.

ICR Guidelines (1978): Establishes that synthetic prescribing must remain subordinate to standard case evaluation. It requires systematic case analysis, group study of Materia Medica, and final confirmation via specific PQRS symptoms.
 
Modern Evolution: Explores how synthetic concepts expanded through authors like Farrington, Otto Leeser, Jan Scholten (Periodic Table), and Rajan Sankaran (Kingdoms & Sensation Method).
 
Survival vs. Disturbed Energy: Highlights that a patient's prescription must balance their pathological expressions ("disturbed energy") with their underlying dynamic coping mechanisms ("survival energy").

Clinical Case Studies

Case 1: Seronegative Spondyloarthropathy (30-year-old male)

Presentation: Polyarticular joint pain with erosions, severe paralysing acute episodes, and a desire to wander.
Analysis: Masked deep anxiety and guilt behind a "happy-go-lucky" facade to avoid feeling like a burden.
Prescription: Synthesised the rigid dutifulness of Kali with the guilt and wandering desire of Bromium, pointing to Kali Bromatum.
Outcome: Marked physical recovery over 2 years, joint destruction halted, and complete social/professional integration.
 
Case 2: Chronic Sinusitis & Severe Varicosities (56-year-old male)
Presentation: Recurrent purulent sinusitis sensitive to drafts, severe varicose ulcers, and sciatica.
Analysis: Marked by childhood emotional abuse, suppressed resentment, and a strict sense of duty toward family.
Prescription: Physical symptoms matched Silicea, but his intense emotional suppression reflected a Natrum taint, leading to Natrum Silicatum 30 to 200c.
Outcome: Long-term relief of physical complaints and resolution of underlying emotional distress.
 
Case 3: Juvenile Idiopathic Arthritis (Young male)
Presentation: Disabling arthritis for 6 years.
Analysis: Experienced severe domestic abuse, viewing his alcoholic father's presence as a daily "thunderstorm" in the house.
Prescription: Ericaceae family themes combined with passive resistance during a storm led to Rhododendron.
Outcome: Significant clinical recovery within 6 months.

Dr Nimish Mehta is a homeopathic consultant, academician, and researcher with over 38 years of experience. Trained under Dr M.L. Dhawale at the Institute of Clinical Research (ICR) in Mumbai, he has served as an Associate Professor at CMP Homoeopathic Medical College and Chief Medical Officer at the Dr M.L. Dhawale Memorial Homeopathic Research Centre. He specialises in rheumatology, respiratory disorders, and chronic complex cases.
